DEVELOPMENTAL SCREENING

Thirty months to thirty-six months old

♥   If your child is thirty to thirty six months old, please answer the following questions with a yes or no response:

  1. Can your child stack eight or more small blocks on top of each to make a tower?   (If she has never tried this, select no.)  
  2. Can your child hop on one foot for at least two or more hops?
  3. When your child talks to people who do not know him will, do they usually understand at least half of what your child is saying?
  4. When you show your child a picture of a horse, cat, dog, person, bird, can they name at least four pictures correctly? (Say no if your child makes an animal sound, like barking or meowing, instead of naming in the animal.)
  5. Can your child wash and dry her hands well enough so you do not have to do them over?   (If you do not allow her to wash and dry her hands by herself, select no.)
  6. If you ask your child to name a friend, would they name another child who is not a member of the family or a pet?
  7. Can your child undress completely without any help?
  8. Will your child join you in singing songs and nursery rhymes?
  9. Does your child express himself in three-word sentences?
  10. Can your child copy a circle?
  11. Does your child know if he is a boy or girl?
  12. Does your child understand right from wrong?
  13. Can your child pedal a tricycle?
  14. Can your child understand directions using the words “on”, “in”, “or “under”? For example, “Get the ball under the table?”
