DEVELOPMENTAL SCREENING

Sixteen to eighteen months

♥   If your child is sixteen months, please answer the following questions with a yes or no response:

1. If you give your child a pencil and will he scribble on a piece of paper without you moving his hand or showing him what to do?   If he bangs or mouths the pencil, select no.   Select yes only if he scribbles without any help.
2. Does your child say two or more words other than “mama” or “dada” and names of family members or pets?
3. Can your child hold a cup or glass by herself and drink from it without spilling too much? (The cup should not have a lid or spout.)
4. Does your child do things to help you, such as picking up his toys or bringing something to you when asked?
5. Can your child carry a large toy or several toys while walking?
6. Can your child pull a toy behind her when walking like a wagon or other type of pull toy?
7. Does your child like to look at picture books and turn the pages by himself?
8. Does your child show her emotions like being happy or sad?

♥   If your child is seventeen months, please answer the following questions with a yes or no response:

1. Does your child like to push, pull, and dump things?
2. Can your child hold a cup or glass by herself and drink from it without spilling much?   The cup should not have a lid or a spout?
3. Does your child do things to help you, such as picking up his toys or binging something to you when asked?
4. Does your child try to talk and repeat words?
5. Does your child say three or more words other than “mama,” “dada,” or the names of family members or pets?
6. Can your child walk all the way across a large room without falling or wobbling from side to side?
7. Can your child dump something small such as a raisin or piece of cereal from a small bottle, glass or cup?   If she has not had the opportunity to try this, select NO.
8. Does your child scribble spontaneously?

♥   If your child is eighteen months, please answer the following questions with a yes or no response:

1. Can your child follow two directions?
2. Can your child build a tower with three blocks?
3. Does your child only put food or utensils in their mouths?
4. Does your child like to climb and take things apart?
5. Does your child try to put on his shoes?
6. Does your child say at least six words?
7. Does your child like to help you or other family members?
8. Does your child run?
