DEVELOPMENTAL SCREENING
Four to six months
♥ If your child is four months old, please answer the following questions with a yes or no response:
- Does your baby sit with his back straight when supported with pillows or a chair?
- Does your baby open her hands and play with her fingers?
- When you stand your baby up, holding her under the arms, does she try to stand on her feet firmly?
- Does your baby laugh out loud?
- Have you seen your baby stare at his own hand for at least five seconds?
- While your baby is on his back, move your hand from one side to the other above his face. Did your baby watch your hand by turning his head from one side all the way to the other side?
- When your baby wants something out of reach, does your baby try to get it by stretching his arms or body?
- Does your baby recognize the bottle and familiar faces?
- Does your baby make happy, excited, or high pitched squealing sounds which are not crying?
- Can your baby hold an object, like a rattle, for an extended period of time?
- When alone, does you baby make sounds other than crying?
♥ If your child is five months old, please answer the following questions with a yes or no response:
- Does your baby vocalize when they are alone instead of crying?
- Does your baby smile and coo to themselves in a mirror?
- Can your baby focus her eyes on small objects the size of a pea or a raisin?
- Does your baby reach for and hold objects with both hands?
- Has your baby rolled over at least two times, from stomach to back, or back to stomach?
- Does your baby like to play peek-a-boo?
- Does your baby stretch out her arms to be picked up?
- Can your baby push themselves up on extended arms when lying on a flat surface?
- Does your baby turn toward new sounds, like when someone enters the room or begins speaking?
- Does your baby smile at others?
♥ If your child is six months old, please answer the following questions with a yes or no response:
- Can your baby feed herself a cookie or cracker? Mark no if she has never been given one.
- Without being propped up by pillows, a chair, or a wall, can your baby sit by herself for at least 10 seconds?
- When your baby is playing and you come up quietly behind him, does your baby usually turn his head as though he heard you?
- Can your baby pick up a toy if it is placed within her reach?
- Does your baby try to copy the sounds you make with your speech like kissing, coughing or saying a word?
- Does your baby grasp and hold his own feet?
- Follow directions carefully. Hold a tissue or some other soft object up high and shake or wave it. When your baby is looking at it, drop it to the floor. Did your baby look down to see where it went?
- Does your baby persistently try to reach and pick up objects that are out of his reach?
- Does your baby move toys from one hand to the other?
- Does your baby smile at other people?
- Does your baby babble and laugh out loud?
