DEVELOPMENTAL SCREENING

Birth to three months

♥   If your child is between the ages of birth to one month, please answer the following questions with a yes or no response:

  1. Does your baby turn his head from side to side?
  2. When your baby is lying on her back, does she look at you and watch your face?
  3. Does your baby move his arms and legs energetically, each of them as easily as the other? Select no if your baby makes jerky or uncoordinated movements with one or both of his arms or legs.
  4. Does your baby like to be held and rocked?
  5. Is your baby gaining weight and growing?
  6. Can your baby suck and swallow easily?
  7. Is your baby startled by loud noises?
  8. When you touch your baby’s cheek, does he turn his head in the direction of your touch?
  9. Is your baby able to raise her head from a surface when she is lying on her tummy?

♥   If your child is two months old, please answer the following questions with a yes or no response:

  1. Does your baby smile and coo?
  2. When you smile and talk to your baby, does he smile back at you?
  3. Does your baby respond (with eye movements, breathing changes or activity changes) to a new sound outside his line of vision?
  4. Can your baby lift their head to at least a 45 degree angle from the floor?
  5. Does your baby roll part of the way to his side when lying on his back?
  6. Does your baby grunt and sigh?
  7. Can your baby hold a rattle for 5 to 10 seconds before dropping?

 ♥   If your child is three months old, please answer the following questions with a yes or no response:

  1. Does your baby reach for objects with both hands?
  2. Can your baby hold an object, like a rattle, when placed in his hand?
  3. Does your baby sleep 8 hours at night without a feeding?
  4. Does your baby rest on her forearms while lying on her stomach?
  5. Does your baby babble, squeal, and coo?
  6. Can your baby hold his head upright?
  7. Does your baby make vowel sounds like “ ooo” and “ ahh”?
  8. When on her stomach, can your baby lift her head and chest to look straight ahead at a ninety degree angle?
  9. Can your baby follow a moving object with her eyes?
  10.  Does your baby turn her head toward bright lights or colors?
  11.  When lying on her tummy, can your baby roll from her tummy to her back?
